Community Project
Without financial resources it is impossible to perform the
work of the organization. The
manifestation of the mission relies on the financial health of the
organization. One way of program funding
is through grants and contracts. Develop a proposal program stemming from the
parent organization (I WILL SEND INTERVIEW).
For example, I met with a domestic violence program leader
and based on the interview you may discover they need x,y,z program. You need to develop a program proposal for
this particular proposed program.
START WITH RESPONDING TO THE REQUEST FOR PROPOSAL
1. Agency
Description: In one paragraph, describe your proposed organization, agency,
program, or community mission and vision.
Be sure to identify the sector in which your proposed organization is
classified. In addition, identify your
organization’s target population and totlal number of clients or participants.
2. Organizational
Structure
a. Attach
organization chart to the application including all the programs of the
proposed organization/program. (I WIL DO)
b. Include
a list of proposed titles and salaries for all proposed employees. Provide a brief job description for each
position (in b) including knowledge, skills, and abilities (IW ILL DO)
c. Attach
project budget to the application. List
other current sources and levels of funding support for the
service/project/program. Each group
member will need to develop an Memorandum of Understanding (MOU) or Memorandum
of agreement (MOA) to clarify commitment of stakeholders. MOA and MOU provides identifying
persons/contacts within the community/organization or outside who are willing
to contribute to the project (in kind contributions, facility space, supplies,
etc. (MOA or MOU would service as a commitment of support..(I WILL DO)
For the following 6 sections, complete for the
service/project/program for which your organization is seeking funding. This does not pertain to your entire
organization unless your organization only offers one service/project/program.
3. Service
Description
a. Describe
in detail the specific activity/service/project for with the agency is seeking
funding
b. Describe
the need for the project, the affected population or community, and the gaps
that your agency proposes to address.
c. Describe
how your proposed project will maximize the target population’s access and
utilization of the proposed services
d. Describe
how the service is unique or innovative
4. Target
population and measurable outcomes
Indicate the following
a. Target
population of this service/project/program
b. Number
of clients/users you anticipate receiving the service and the duration and
frequency of services planned in FY.
c. Admission
and termination procedures for the project and whether your agency has a
termination policy in place
d. Specify
the _____ department (include the goals or businesses plan section) that your
services/project supports
e. Measureable
client/user outcomes (improvements) anticipated as a result of receiving services, and the relationship of these
outcome to the _____ goal that your proposal addresses.
f.
Strategies for achieving the outcomes
g. How
the outcomes will be measured
h. The
follow up that your agency will do to determine the long term impact of your
project.
5. Organization
resources
a. Describe
the required facilities, equipment, and other resources necessary for the
implementation of the project and your agency’s access to these resources.
b. Describe
the agency’s ability to provide this service if _____ funds are not
available. What other
agencies/organizations/stakeholders are willing to provide support
6. Collaborative
effects
a. Describe
how the proposed project will be cooperatively or collaboratively managed with
other organizations (e.g. Monthly
meetings, committee development, task force, creation etc.
7. Measure
impact
a. Research
local, state, national and international similar projects or projects of
similar magnitude and discuss the outcome performance of these programs.
8. Contract
amount (I WILL DO)
a. Indicate
the proposed contract amount for _____ to enter into with the agency.
b. Using
the sample format attached and below, indicate specifically how the funds will
be used (i.e salary etc.
c. Attach
project budget to the application. List other current sources and levels of
funding support for the service/project/program. Each group member will need to develop an MOU
or MOA to clarify commitment of stakeholders.
